Faulty engineering caused the catastrophic implosion of the Titan submersible, which was en route to the Titanic wreckage and resulted in the instant deaths of all five occupants. The National Transportation Safety Board (NTSB) concluded in its final report that the submersible's carbon fiber composite pressure vessel had multiple flaws and did not meet necessary strength and durability standards due to poor engineering. OceanGate, the submersible's owner, also failed to adequately test the Titan and was unaware of its true durability.
The NTSB report echoes findings from a prior Coast Guard investigation, which deemed the implosion preventable and described OceanGate's safety procedures as critically flawed with glaring disparities between protocols and practices. The company, which had been making voyages to the Titanic site since 2021, suspended operations and wound down following the incident. The implosion also killed OceanGate CEO Stockton Rush, along with Paul-Henri Nargeolet, Hamish Harding, Shahzada Dawood, and his son Suleman Dawood.
